Pickup Location for St. Claire HealthCare Medical Records Temporarily Relocated

Due to a temporary relocation of St. Claire HealthCare’s (SCH) Health Information Management (HIM) Department, patients who need copies of their medical records will be visiting a new location inside St. Claire Regional Medical Center. A representative from the Health Information Management team is now located in the registration area just inside the hospital’s main entrance to provide assistance, Monday thru Friday from 8 AM to 4:30 PM.

SCH’s HIM team manages requests for printed copies of medical records, as well as diagnostic images (x-rays, CT scans, MRIs, etc.). While most medical records are easily accessible from your computer or mobile device through your MyHealth Patient Portal (www.st-claire.org/myhealth), printed copies can be requested in person or by calling 606.783.6570. A completed “Release of Information” form is required before any requests will be processed and requests are generally completed within seven to 10 days.
